Scattered-Site Apartment Program

Scattered-Site Apartment Program residents live in transitional or permanent private apartments throughout Brooklyn, depending on their care needs. Our case managers travel to the residents’ homes regularly to help them learn how to live on their own and thrive.

Targeted services available to Scattered Site residents include substance abuse and mental health counseling, daily living skills training, medication management, health and wellness support, and participation in recreational and therapeutic activities at our various housing sites.

Brooklyn Housing Assistance Center (BHAC)

BHAC is an eviction prevention initiative that helps housing-vulnerable adults and families in Brooklyn to maintain their housing and avoid ending up on the street or in shelter. Participants must demonstrate that they are at imminent risk of losing their housing.

Case managers provide rental arrears assistance, utility cut-off notices, apartment searches, legal assistance, landlord mediation and more. Individuals are also eligible to participate in other BCHS services including case management and counseling.
