ABOUT THE APARTMENTS:

The Transitional Apartments serve 63 formerly homeless adults, each placed private Brooklyn apartments for up to 36 months.

93% (58 of 62) of transitional residents maintained their housing or moved on to more independent living last year (FY 2014).

The Supported Apartments provide long-term housing to 72 formerly homeless individuals.

98% (65 of 66) of residents maintained their housing successfully or moved on to more independent housing this year.

SCATTERED-SITE APARTMENTS

The Scattered-Site Apartment Programs place 135 adults with serious and persistent mental illnesses into private apartments in neighborhoods throughout Brooklyn. Residents receive the intensive support they need to live independently, including on-going case management, daily living skills training, substance abuse counseling, and mental health care.

The program offers mental health counseling, medication management, daily living skills training, substance abuse services, health services, recreational activities, and the development of socialization and self-advocacy skills.
